Once we receive your request, you will receive an email within 24 hours to confirm your appointment date and time.

*If you are scheduling a school physical and the child has been vaccinated in another state, please provide a copy of their out-of-state records before the appointment date. This allows vaccinations to be transferred into the mandatory Florida SHOTS database, and provide the Florida Form 680, required for school registration.
